Miller DH-AP-12 anchor plate is designed for permanent bolting to concrete and other structures.

• Use only with portable fall arrest masts & accessories approved or manufactured by Miller
• Ratings, design factor and capacities may vary with different system components & configurations. See specification data sheets for system components to stay in compliance with requirements
• Mounting surfaces must stay within 15 degrees of horizontal & plate needs to be installed so the fall arrest mast may be leveled prior to use
• Every installation need to be approved to the local standards by qualified engineers
